Remote patient monitoring and telehealth increases access to care and dramatically improves the way we monitor and support patients outside of traditional clinical settings. But for telehealth to deliver these benefits, a seamless flow of data between providers, patients, third-party applications, and medical devices is necessary. With Redox, keeping track of at-risk patients and making the information available in any EHR for review by medical professionals has never been easier. 

Example workflows we support include:

  • Trigger alerts or care interventions based on vitals/responses to outreach
  • Write discrete clinical values (vitals, glucose levels, etc.) captured by medical devices back to a patient's chart
  • Send summary reports of actions taken or vital levels over time in the form of PDFs to the patient's medical record

We make it easy to retrieve complete medical histories so caregivers have all the information they need to accurately diagnose and administer care.

Common information we retrieve prior to an encounter includes:

  • Patient allergies
  • Patient medications (past, current, and future)
  • Family history (relation, problems, cause of death)
  • Current immunization status and pertinent immunization history
  • Problems (patient's past and current relevant medical problems)
  • Any medical equipment that the patient uses or has been prescribed
  • Past results from laboratories, imaging procedures, and other procedures
  • Social history (tobacco use, pregnancies, and generic social behavior observations)
  • Summary of past clinical encounters (type of visit, location, attending provider, associated diagnosis)
  • Summary of past procedures (diagnostic procedures, procedures that change the body, and services performed by clinical staff)
  • Plan of care (future appointments, medications, orders, procedures, and services that a patient may be scheduled for or is waiting to be scheduled for)

Every RPM or telehealth visit needs to be documented and available for reference by the patient's primary care team. Often, telehealth visits lead to follow up tests or referrals to specialty providers. We make it easy to document the encounter details in any EHR and schedule follow up events. 

Common supported workflows at the conclusion of a telehealth visit include: 

  • Order follow up tests (imaging, labwork, etc.)
  • Schedule follow up visits with PCP or specialty provider
  • Store images used as part of the diagnosis or care event
  • Integrate the encounter documentation in the patient’s record at the EHR where the patient primarily receives care

Utilizing our Redox Scheduling API, we can update you any time an appointment is created for your RPM or telehealth service and allow patients to directly schedule appointments with providers. We integrate with existing scheduling systems to log events as they occur and exchange necessary patient demographics to provide and document care.
